ACT 34 PENNSYLVANIA STATE POLICE CRIMINAL RECORDS CHECK
All coaches shall submit the results of the Act 34 Criminal Record Check from the PA State Police. The check can be done online and typically is available immediately.

ACT 151 PENNSYLVANIA CHILD ABUSE HISTORY CLEARANCE
All coaches shall submit the results of the Act 151 PA Child Abuse History Clearance. The check can be done online and typically available within a week.

FBI FEDERAL CRIMINAL HISTORY CLEARANCE
Coaches shall submit the results of FBI Federal Criminal History Clearance, which included the fingerprint search. The coach may submit an exemption affidavit if the coach has been a continuous resident for the entirety of the previous 10 year period. ARREST/CONVICTION REPORT AND CERTIFICATION FORM
Coaches shall submit an executed Arrest/Conviction report and certification form. A blank form is attached to this packet.

EVIDENCE OF COMPLETION OF THE MANDATED REPORTER TRAINING
Coaches must complete the mandated reporter child abuse training program and obtain the certificate of completion.
